This Apollo Privacy Notice (“Privacy Notice”) helps explain the information practices of Gauss Surgical and its affiliates (“Gauss Surgical”, “we”, or “us”) when you use our COVID-19 testing eligibility tool, along with our related mobile application “Apollo” and web application and website at covid.operationappolo.com (collectively, the “App”). This Privacy Notice supplements and is subject to Gauss Surgical Privacy Policy (“Privacy Policy”), which applies to the use of all of our Services (as defined in our Privacy Policy), including the App. Please read the Privacy Policy for additional information about our information practices.

Registration Information. If you use our App, we ask you for your phone number and yourresponses to a questionnaire. If you are a medical provider using our App, we will ask you for your business email address.
